All Care Dental collects and keeps personal health information so that we can treat you safely. This page explains what we collect, why, and what you can ask us to do with it.
What we collect
Your name and contact details, your dental and relevant medical history, the medications you take, your insurance details, clinical notes, X-rays and photographs taken as part of your care, and a record of appointments and payments.
Why we collect it
- To examine, diagnose and treat you, and to plan future treatment.
- To bill your insurance plan and to process payments.
- To contact you about appointments and follow-up care.
- To meet the record-keeping requirements that apply to dental practices in Ontario.
Who we share it with
We share information only where it is needed for your care or where the law requires it: with your insurance plan when we bill on your behalf, with a specialist or laboratory involved in your treatment, and with public health or regulatory authorities where they have a legal right to it. We do not sell personal information, and we do not use it for advertising.
How it is kept
Records are held in the practice management system and in locked storage at the office, with access limited to the people who need it to do their job. Records are kept for the period required of dental practices in Ontario and then securely destroyed.
Your choices
- You can ask to see your record and to get a copy of it.
- You can ask us to correct anything that is wrong.
- You can ask how your information has been used or disclosed.
- You can withdraw consent for a use that is not required for your care or by law.
This website
This site does not set advertising cookies or track you across other sites. The map on the contact page is embedded from Google Maps, which may set its own cookies when it loads. The appointment form is a demonstration and does not currently send or store anything.
